Appointments

Patients now need to submit an online form to request a doctors appointment.

Please click on the green banner below which says 'contact us online' to request an appointment.  If you are unable to complete the form yourself, please call reception who will complete the form on your behalf.

Once the form has been submitted, the form will be triaged by the doctor and you will be contacted with an appointment suitable to the clinical need of your problem.

You can submit a form Monday to Friday 7am to 7pm. All forms submitted before 2pm will be triaged on the same day. Forms submitted after 2pm may be dealt with on the next working day.

For all other healthcare professional appointments such as a nurse, please ring our main switchboard number on 0113 2310626 and press option 1.

If you are newly pregnant & need to book antenatal care, please contact reception who will help you arrange an appointment with the midwife.

You can help us by being on time for your appointment and letting us know in good time if you can’t attend. It is practice policy that anyone who fails to attend 3 or more appointments within 12 months will asked to register with another practice in the area. We use a free text message appointment reminder service. Patients not wishing to receive these reminders can opt of out this facility at any time.
