14th July 2021 - Masks and Social Distancing
You will have seen in the media that the government have announced that the wearing of masks in an enclosed space will no longer be a legal requirement from the 19th July. However, we would like to notify all patients that the wearing of masks and the need to maintain a social distance will still be required to enter our building, around our building and in all consultations, unless exempt. We appreciate your cooperation.
You can choose whether your confidential patient information is used for research and planning. You do not need to do anything if you are happy about how your confidential patient information is used. You can change your choice at any time.
Type 1 opt-out: medical records held at your GP practice - You can tell your GP practice if you do not want your confidential patient information held in your GP medical record to be used for purposes other than your individual care. This is commonly called a type 1 opt-out. This opt-out request can only be recorded by your GP practice. Please fill and return this form to the surgery Type 1 Opt Out form
Type 2 opt-out: information held by NHS Digital - Previously you could tell your GP practice if you did not want us, NHS Digital, to share confidential patient information that we collect from across the health and care service for purposes other than your individual care. This was called a type 2 opt-out. The type 2 opt-out was replaced by the national data opt-out. Type 2 opt-outs recorded on or before 11 October 2018 have been automatically converted to national data opt-outs. Due to COVID we are still only able to offer telephone triage with a GP rather than pre-bookable face to face appointments. You will need to ring on the day to be given a telephone appointment. If the Doctor or Nurse can not deal with your problem ove r the phone they may ask you to come down. Please do not come into the practice to book an appointment.

The Armley Hub
From December 2016 if you need to see a GP but can’t attend the surgery through the week you can book an appointment at the Armley Hub.
Routine appointments are available to prebook via our Hub service between 07.00-08.00 on weekdays, 08.00 – 14.00 on Saturdays and 08.00 – 12.00 on Sundays. For urgent appointments, the service does also offer same day appointments. To access these appointments please ring the practice as normal. If the surgery is closed a divert is in place to direct patients to the Hub directly during its operational hours.The Hub runs from Priory View Medical Centre.
The Hub was designed to provide better access for patients who struggle to attend the practice through the working week, and involves a group of practices from the local area. All patients from this group of practices can book appointments at the Hub, and you will be seen by a GP who works on behalf of the local practices. This may mean that you have an appointment with a GP who does not usually work for your local practice, but who will be able to give you the same level of care. All of the GPs working in the Hub will be able to access all the necessary information about you to give you the highest level of care and the process will be exactly the same as you would receive from a GP Monday to Friday. Online services - Appointments and Repeat Prescriptions
We want to get better at communicating with our patients. If you find it hard to read our letters or if you need someone to support you at appointments, please let us know. We want to know:
- if you need information in braille, large print or easy read
- if you need a British Sign Language interpreter or advocate
- if we can support you to lipread or use a hearing aid or communication tool
Please tell the receptionist when you arrive for your next appointment, or call on the main surgery number
Named Accountable GP
From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a “named accountable GP”. This GP has responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required, based on the GP’s clinical judgement.
If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery. Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
Please inform us if you wish to change your “named accountable GP”.
Friends and Family Test
From 1 December 2014, if you visit our GP surgery you will be asked about whether or not you would recommend our surgery to your family and friends, if they need similar treatment or care. For more info Click here.
