New Patient Registration

If you live in our practice area and would like to register with us, please come to the surgery with 2 forms of ID and fill out the registration forms and health questionnaire.  Our practice is located in postcode area LS12 1JE.  Our catchment areas are LS1, LS2, LS12 and LS13.

We now offer HIV and Hepatitis B and C screening to all our new patients aged 16-65 at registration. If you agree to the test please tick the consent form at time of registration and hand back to one of our receptionists.

Named Accountable GP

From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a “named accountable GP”. This GP has responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required, based on the GP’s clinical judgement.

If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery. Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.

Please inform us if you wish to change your “named accountable GP”.
