Changes from the 8th March 2024 

Demand on our service is at an all time high and we need to ensure patients with urgent medical problems are dealt with quickly.

Currently once the available book on the day appointments have all gone, any patient who deems their medical problem as urgent for that day is added to an emergency list and a nominated doctor contacts those patients. Often these issues are not medically urgent, they are longstanding conditions, sick notes or repeat prescription requests therefore the doctors want to change the process so we can deal with urgent problems urgently, and routine conditions appropriately.

From the 8th March, once all book on the day appointments have been taken, patients will be asked to fill in an online form. If patients are unable to do this, the receptionist can help. The nominated doctor will then triage these online forms deemed on their medical need. Some patients will be seen that day, some patients will be offered a routine appointment, some patients will be provided with advice.

We have consulted with the patient participation group about the change and we hope this will better the service we offer.


TARGET Training Afternoon - The practice will be closed from 12pm on Thursday 2nd May 2024 - we will reopen at 8am the following day. For medical advice when the practice is closed -

  • When you think you need A&E, go to 111 online first. They’ll help you right away and, if needed, a healthcare professional will call you.
  • You can access NHS 111 online by visiting the website 111.nhs.uk
  • People with life-threatening illnesses or injuries should dial 999.
  • NHS 111 online makes it easier for patients to get the right advice or treatment.
  • NHS 111 online can make sure you access the most appropriate service for your health concern. If needed, NHS 111 online can direct you to an urgent treatment centre/walk in centre, emergency dental service, GP, pharmacy or another more appropriate local service.

Data Op-Outs

You can choose whether your confidential patient information is used for research and planning. You do not need to do anything if you are happy about how your confidential patient information is used. You can change your choice at any time.

Type 1 opt-out: medical records held at your GP practice - You can tell your GP practice if you do not want your confidential patient information held in your GP medical record to be used for purposes other than your individual care. This is commonly called a type 1 opt-out. This opt-out request can only be recorded by your GP practice. Please fill and return this form to the surgery Type 1 Opt Out form

Type 2 opt-out: information held by NHS Digital - Previously you could tell your GP practice if you did not want us, NHS Digital, to share confidential patient information that we collect from across the health and care service for purposes other than your individual care. This was called a type 2 opt-out. The type 2 opt-out was replaced by the national data opt-out. Type 2 opt-outs recorded on or before 11 October 2018 have been automatically converted to national data opt-outs. THE ARMLEY HUB - From 1st October 2022 if you need to see a GP but can’t attend the surgery through the week you can book an appointment at the Armley Hub on Saturdays 08.00 – 14.00 and it runs from Priory View Medical Centre. For urgent appointments, the service does also offer same day appointments. To access these appointments please ring the practice as normal. If the surgery is closed a divert is in place to direct patients to the Hub directly during its operational hours. The hub was designed to provide better access for patients who struggle to attend the practice through the working week and involved a group of practices from the local area. All patients from this group of practices can book appointments at the hub, and you will be seen by a GP who works on behalf of the local practices. This may mean that you have an appointment with a GP who does not usually work for your local practice, but who will be able to give you the same level of care. All of the GPs working in the hub will be able to access all the necessary information about you to give you the highest level of care and the process will be exactly the same as you would receive from a GP Monday to Friday. Please remember the hub is an extension of general practice as a way to provide more GP appointments to the local population. For more information about the Armley Hub, click here.

Accessible Information

We want to get better at communicating with our patients.  If you find it hard to read our letters or if you need someone to support you at appointments, please let us know. We want to know:

  • if you need information in braille, large print or easy read

  • if you need a British Sign Language interpreter or advocate

  • if we can support you to lipread or use a hearing aid or communication tool

Please tell the receptionist when you arrive for your next appointment, or call on the main surgery number


Named Accountable GP

From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a “named accountable GP”. This GP has responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required, based on the GP’s clinical judgement.

If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery. Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.

Please inform us if you wish to change your “named accountable GP”.


Friends and Family Test - From 1 December 2014, if you visit our GP surgery you will be asked about whether or not you would recommend our surgery to your family and friends, if they need similar treatment or care. For more info Click here.
